Rhinoplasty improves the look and proportion of the patient’s nose, enhancing facial harmony and self esteem. Nose surgery may also correct impaired breathing caused by structural abnormalities in the nose. It is recommended to wait until 14 or 15 years old, and possibly older for boys, before undergoing rhinoplasty as the nose may not be fully developed at a young age. There is no upper age limit for nose surgery. Rhinoplasty can be performed in conjunction with a facelift or other rejuvenation procedures to correct ageing of the nose. Certain breathing problems related to the internal nasal structures can be corrected while nose surgery is performed.
How Nose Job is Performed
Cosmetic nose surgery can be performed under local or general anaesthesia depending on the size of the nose, patient’s and surgeon’s preferences. Moderate sedation is used on local anaesthesia which numbs the nose and the surrounding area. Patient is awake during the procedure, but will have a comfortable and painless surgery. Patient sleeps when general anaesthesia is applied.
Aesthetic nose surgery usually takes an hour or two. During surgery, nose skin, bone and cartilage will be devoted from the roof of nose. Process of reshaping the nose differs depending on the technique applied and the current problems surrounding the shape of the nose. The skin will be adapted to newly created nose roof.
Most of the plastic surgeons perform the aesthetic nose surgery inside of the nose. Some of them, especially in complicated cases, would prefer an open technique. They make a small incision on vertical strip of tissue (columella) which separates the nostrils. When the surgery is completed, a plaster splint will be applied to the patient’s nose in order to help retain the new shape of the nose. Bumpers or soft plastic devices can be placed into the nose to secure the septum, the part that separates the two air paths.
Results
During the first 24 hours after surgery, the patient feels her face is swollen. It is normal to have pain on the nose and the head area. Painkiller prescribed by the surgeon can soften the patient’s discomfort. The patient should lie down on the bed with her head up. Bruising and swelling around the eyes becomes most prominent after 2 or 3 days. Application of cold compresses reduces swelling and makes patient feel a little better. Most of the swelling and bruising disappears within the first few weeks. Patients generally stand up in two days after cosmetic nose surgery and they can return to school or business life one week after surgery though they are advised not to be too active in this period. In general, activities that require intense exercise (running, swimming, gymnastics, movements that raise patient’s blood pressure) should be avoided for two to three weeks. Day after day, patient’s nose will begin to look better and patient’s self-esteem will rise gradually. Patient will not look like as if they have just undergone a new surgery. However, healing would still be slow and a gradual progressive process.
Limitations
There are some limits with the amount of change that can happen with rhinoplasty. These are the length and the width of the nose (narrowed or widened), and techniques that can be performed. It is very significant to make sure that the nose balances with itself and rest of the face. It is impossible to take a very large nose and make it very small and still have it look well shaped, balanced, and matching with the person’s face.
Risks
Complications of aesthetic nose surgery are rare and usually minor when performed by an experienced plastic surgeon. Nonetheless, there is always a possibility of complication. These may be infections, bleeding or reaction to anaesthesia. After surgery, cracked small capillaries on the surface of the skin may occur in the form of very fine red spots. Since cosmetic nose surgery is performed inside the nose, there is usually no visible scar. However, when open technique is used or narrowing of the wide nostrils is necessary, very small and thin scars may appear on the base of the nose. Second surgery may be required in 1 of each 10 cases. It is not possible to predict, and even the most skilled and experienced surgeons may need to perform second surgery. The corrective surgery is a simple one compared to the initial surgery.
Contraindications
Rhinoplasty can not be applied to every patient. Contraindications to rhinoplasty include severe co morbid circumstances (e.g. heart disease, diabetes, wound healing disorder, excessive scarring, excessive cigarette smoking, pregnancy, history of thromboembolic disease). Patients with inappropriate motivations for seeking rhinoplasty and unrealistic expectations are discouraged from undergoing a rhinoplasty.
