He/she will make some preparations beforehand to take care of any emergency that may arise. You could argue that you can just tell the doctor about your health status. This is risky. You might forget to inform the surgeon about a particular thing during your consultation. So, you must fill out your medical history form beforehand.
It can happen that due to a certain medical condition, our surgeon might deem you unsuitable for surgery. Knowing your medical history also helps the surgeon decide on the kind of medications you can take and how many doses you will need. They might also prescribe you some other medications due to your condition, which can help support your recovery.
Many people develop an allergic reaction to certain foods, medications, dust, pollen, etc. Through your medical history form, our surgeon will make sure to not prescribe you any medicine that you’re allergic to.
When Should You Submit Your Medical History Form?
At Longevita, we will ask you for your medical history form after you’ve booked your treatment with us. You’re advised to fill it out as soon as possible so that we can find out if everything’s okay or not. You will need to submit it before booking your flights to Turkey. That’s because our surgeon needs to confirm if you’re suitable for a particular surgery. He/She might suggest that you stop a certain medication before your surgery if you are on any medication.
You should also know that during the initial consultation session, our Patient Consultant will ask for your brief medical history. They’ll ask about any diseases that you have or any medications that you’re currently taking and share them with the surgeon. If you have any condition that stops you from having the surgery you want, you’ll be informed accordingly.
It’s better if you write the important things down about your medical history. This way, you reduce the chances of forgetting anything important. You can also keep on adding information to the list after you’ve gathered it from your close relatives and family members.
What Kind of Questions Will You Answer in Your Medical History Form?
In your medical history form, you’ll have to tell your age, date of birth, height, and weight. You’ll then go on to answer questions, such as if you’re currently receiving any medical treatment and if yes, which one? You should know about the prescribed medications that you’re taking because you’ll have to mention them in the form. The form will also inquire if you carry a medical warning card, are pregnant, have any allergies (food, medications, environmental, or product material, among others), or have hay fever.
The medical history form will also require information from you about any sickness, heart problem (if you’ve had heart surgery), liver or kidney diseases, epilepsy, diabetes (including a family history of diabetes), and if you suffer from any fainting attacks. The form will also contain the question if you’ve ever tested positive for infectious and contagious diseases, such as HBsAg, HCV, HIV or COVID-19.
Questions regarding previous injuries, past hospitalisations (reasons for them, their dates, duration, and treatment), recent illnesses, and family history (of cancer, cardiovascular problems, diabetes, psychological problems, etc.) are part of the medical history form.
Some of the questions are important in regards to the surgery. You’ll have to mention if you’ve ever had a bad reaction to local or general anaesthesia in the past. Moreover, we need to know if you have any dermatologic problems related to your scalp, including eczema, dandruff, chronic rash, folliculitis, fungal infection, psoriasis, and dermatitis. These are especially important if you’re getting a hair transplant in Turkey.
We also need to know if your blood has ever been refused by the blood transfusion services. And some of the questions will be about your lifestyle, such as the number of cigarettes, if any, you smoke per week and/or the number of alcohol units you consume per week.

Who Will Read Your Medical History Form?
Complete honesty is of the utmost importance when you’re answering the questions in your medical history form. Otherwise, you can mislead our medical team, which can adversely affect your health. You need to be open with us, and you shouldn’t hide anything.
After you submit your medical history form, your surgeon(s) will confirm your suitability for the surgery. If you’re suitable, you will be notified via email. After that, you can proceed to book your surgery date and flight tickets.
How Will the Medical History Form Answers Impact Your Prospects of a Surgery?
If you have any dermatologic problems such as eczema or dandruff on the scalp, your hair transplant aftercare might be a bit more challenging. However, we will make sure to guide you about this during the recovery process. Before the procedure even takes place, we will make sure to inform you about all the possible risks that come with the surgery due to your condition.
You’ll have to submit information about the medications that you take regularly. Some medications are not suitable for you to take before or after the surgery. Certain medicines can repress your immune system or negatively impact it.
So, after the surgery, you’re more prone to developing an infection because of those medicines. Your weakened immune system might be unable to fight the infection. This is a highly risky situation, so we must know in advance about it. Once we know what you’re taking, we might ask you to avoid taking them for a certain time period.
Your surgeon might also advise you to conduct your own doctor at home. You might need to get written consent from your doctor for the surgeries you wish to have. If they don’t advise you to have a cosmetic procedure, we are also not able to operate on you.
The reason why we’re asking about your weight is that some cosmetic procedures cannot take place on patients who’re overweight. In this case, we’ll either cancel the surgery or ask the patient to lose weight before the surgery takes place.
Why You Should Provide Us with Correct Information?
Ensuring your good health before, during, and after the surgery is extremely important to us. If you do not provide us with the correct information, you might have some complications during and/or after the surgery. Unfortunately, some patients are unsuitable for surgery. We will discuss other treatment options with you if this happens.
We will perform some blood tests before the surgery once you arrive in Turkey. So, our surgeons will find out if you can or cannot have a treatment. If you’ve provided incorrect information and the surgery cannot take place, we must cancel your treatment and you will be liable for cancellation charges.
